Guam Wage and Tax Statement. This form is used by employers for employees for whom the employer withheld Guam income tax or United States Social Security and Medicare taxes, those that the employer would have withheld Guam income tax if that employee had not claimed more than one withholding allowance, or those whom the employer paid any amount for services if they are in a trade or business.
